THIS IS NOT AN INVITATION FOR BID, PLAN AND SPECIFICATIONS ARE NOT AVAILABLE Small Business Set-Aside PROJECT NUMBER: ND OMAD 2025(1) PROJECT NAME: Minuteman Missile Access PROJECT DESCRIPTION: The Office of Missile Air Defense Program (OMAD) is a Defense Access Road (DAR) program used by the Department of Defense (DoD) to fund the annual maintenance and improvements on certified gravel roads. FHWA works with the Military Surface Deployment and Distribution Command (SDDC) to support the Air Force Intercontinental Ballistic Missile Program (Minuteman). These Minuteman Missile Access Roads are located in 6 counties within North Dakota. SCOPE OF THE WORK: The work includes roadway reconditioning and adding four inches of aggregate surface course as well as the replacement of up to 27 culverts. All proposed work is to stay on the existing roadway bench. The proposed typical section consists of reconditioning the existing gravel roadway per Section 303 and adding an additional 4-inches of Aggregate Surface Course with a maximum width of 24-feet. A 4% crown is proposed or as approved by the CO. The Contractor will need to taper down the proposed aggregate surface, per the typical section, at applicable locations. The project will be procured under the FPVS (Fixed-Price Variable Scope) contracting method and includes approximately 50 miles of variable scope roadway reconditioning and re-graveling, and up to 27 culverts of variable scope in length. The project PS&E will provide a prioritized schedule of roadways to be improved that includes roadway reconditioning and re-graveling of existing aggregate surface roads within the existing roadway bench; as well as the replacement of up to 27 culverts and subgrade repairs. The Indian Health Service Headquarters is located in Rockville, MD and then divided into twelve (12) physical areas: Alaska, Albuquerque, Bemidji, Billings, California, Great Plains, Nashville, Navajo, Oklahoma, Phoenix, Portland, and Tucson. The Great Plains Area Office (GPAO) works in conjunction with 19 Indian Health Service Units and Tribal Managed Service Units to provide health care to approximately 130,000 Native American located in North Dakota, South Dakota, Nebraska, and Iowa. QNBMHCF falls under the GPAO and provides medical care to approximately 14,550 tribal members living on the Turtle Mountain Indian Reservation. The Service Unit was built in 1967 with renovation and clinic expansion completed in 1992, to provide an expanded level of outpatient health care services specifically designed to meet the needs of our population. Services include: 24 Hour Emergency Services, Laboratory, Radiology (CAT Scan & Mobile MRI) and 24-Hour Pharmacy Services, Optometry, Dental, Clinic, Emergency, Podiatry, Same Day Clinic, and Behavioral Health. 2.0 SCOPE Provide QNB with blood and blood products. These products will be used to provide direct patient care in our ER, OR, and OB wards. This firm-fixed-price Purchase Order will be for one base year and 4 option years. Vendor will ship out new Red Blood Cell (RBC) stock every two to three weeks. Old stock will be shipped back to the vendor when new stock arrives. Plasma and Platelets will be sent on an as needed basis. Please note that if NYCHA receives no responses to this RFQ, the bid submission deadline shall be automatically extended for seven (7) days. This does not limit NYCHA's right to extend the bid deadline for any other reason. Current Prevailing Wage Links are posted on iSupplier under the Quick Links Section of Sourcing Supplier.  The vendors are responsible for remaining informed of all updates to the Prevailing Wage Schedule.
